The authors report a case of a mate, 19, white patient that complained of intermittent claudication in the right limb since 20 days. At physical examination, all puises of the right leg were absent and the skin was pale. At doppler examination the ankle-brachial index was- 0,56 in the rigth limb, and was seen a diminution of the blood flow after passive dorsiflexion of the foot. A popliteal artery entrapment syndrome was suspected and the patient was undergone to femoral angiography that confirm popliteal occlusion in the rigth leg and disclosed arterial narrowing in the left leg during active plantar flexion. The left limb was undergone ta surgical exploration that disclosed an aberrant lateral attachment of the medial head of the gastrocnemius muscle causing entrapment of the popliteal artery. The operative procedure was myotomy and the postoperative course was uneventifull. The rigth limb was clinically treated with good results.
