The authors present the frequency of hospital and community infection which was of 10.56%and41.38% respectively at Hospital Universitário Regional do Norte do Parana, in 1988, in a comparative analysis of its topographic distribution and its etiology; agents. The rate of urinary infection was of 17,97% of nosocomial infections and of 8.25% of community infections. In low respiratory infections the rate was of 15.85% when nosocomial and of 25:41% when in community. Surgical skin infections represented 17,89% of the nosocomial infections and other sldn infections were responsible for 18.79% in community infections. Concerning the frequency of etiological agents it was observed that in nosocomial infection, Escherichia coli was responsible for 20.75% of the cases, second was Klebsiella sp, 19.53%, and then Pseudomonas aeruginosa, 16.50%. In community infections Escherichia coli was isolated in 38.40%, Alebsiella sp in 14,40% and Staphylococcus aureus in 12,80% of the cases. Some invasion procedures directly associated with these infections were also analysed. In urinary infection a rate of 1.40% in nosocomial infections, in general terms was observed, and 7.40% when associated with some kind of manipulation of the urinary tract. The same occurred with the nosocomial pulmonary infection, in which rate of 1,64%, in general terms was observed, and when there had been some manipulation of the respiratory tract the rate was of 7.42% of the patients. The authors emphasize a constant concern with all the medical staff and the Infection Control Commission regarding the indication of invasion procedures, and she need of extreme rigor in aseptic and antiseptic techniques, sterilization and invasion procedures.
